Hardtner Medical Center encourages our patients to enroll and use our Patient Portal through Cerner Health.

Our Patient Portal provides these services:

  • Access to your Health Record
  • Management of your Health Information
  • Sending and Receiving Messages

My health record information is missing.

Usually the portal displays only the parts of your health record that are associated with a particular provider. Also, some sensitive information isn’t displayed in the portal. If you still think some of your health record is missing, discuss this with us during your next visit or send us a message from the portal. I am not seeing my laboratory results.

Some results may be considered sensitive, in which case they aren’t displayed in the portal. Your care team can tell you which information is displayed. If you still have questions, contact us at 318-495-3131 ext 856.

My document is missing.

Not all documents in your electronic medical record are displayed in the portal. Your care team can tell you which documents are displayed. If you still have questions, contact us at 318-495-3131 ext 856.

My radiology report is missing.

Not all radiology reports in your electronic medical record are displayed in the portal. Your care team can tell you which radiology reports are displayed. If you still have questions, contact us at 318-495-3131 ext 856.

My pathology report is missing.

Not all pathology reports in your electronic medical record are displayed in the portal. Your care team can tell you which pathology reports are displayed. If you still have questions, contact us at 318-495-3131 ext 856.
